ULTIMA: Skull of Quakes doesn't reveal entrance

Description
In Ultima 8, you can at some point use a 'Skull of Quakes' on a wall opening to open the ground and reveal a teleporter room.
Normal behavior is that this room appears after using this skull.
Right now the behavior is that the ground doesn't open until after you leave the screen and come back, leaving players thinking what that skull was supposed to do.
I've included my savegames, just load the Skull of Quakes save and use the skull in my backpack on the hole in the wall.
It's very minor but felt like reporting it nonetheless.

I can confirm this issue during my play through. It seems we may need to reset fast area after palette fade out.
Whoops, actually, the problem is much worse. I'm pretty sure you're supposed to be teleported to a room where the teleporter resides (when you walk into the door opening), but you're not getting teleported.
Instead you walk past the spot that should have teleported you (on or behind the door) and fall to your death.
Leaving the area completely and coming back again doesn't seem to fix this.
