The one thing I did was "register" the Da Vanci final evidence(The Codex Adlatacis?) twice but I went back and did it a bunch more times and now my score is 4/4 like it should be?

Your saved game looks correct.
I've fixed a corruption of the global flags in death screens. This may be related to what you experience. Since we can't reproduce the issue anymore, this was most likely caused by the memory corruption issue.
