Ok, figured out the "sometimes" part. It's when you position your mouse cursor on the menu, the arrow keys become unresponsive. This is a bit tricky to spot as the mouse cursor is invisible and the menu doesn't respond to clicks. This seems to happen in both windowed and fullscreen modes, even though in fullscreen it's even harder to tell where your invisible mouse cursor actually sits.

I've added a mouse cursor to the UI. If this fixed the issue for you - please let me know.

The original issue is fixed. And I wasn't expecting clicking on the options to actually work so that's also pretty cool. :)

I did notice however, that if you go two levels deep into the Option menu, to Volume settings for example, and then back out using Esc or "Return to previous menu" the mouse cursor vanishes. It still works as do the arrow keys and clicks. It's just not visible. And it's restored again if you go either deeper or higher in the menus.
